The information provided on this website is for general informational and educational purposes only and is not intended as medical, mental health, or legal advice. To the fullest extent permitted by law, Pyxis Counseling, LLC and Laura Heiman, LCSW shall not be liable for any damages arising out of or related to the use of, or inability to use, this website.
This practice shall not be liable for any failure or delay in performance resulting from circumstances beyond its reasonable control, including but not limited to natural disasters, power outages, internet disruptions, illness, or governmental actions.

No Therapist–Client Relationship
Use of this website does not create a therapist–client relationship. A therapeutic relationship is established only after required intake documentation has been completed and services have been formally confirmed.
This practice does not guarantee specific outcomes or results from therapy services.

Electronic Communications
Electronic communications, including email and contact forms, may not be fully secure despite reasonable safeguards. By communicating electronically, you acknowledge and accept potential risks to confidentiality. Sensitive or clinical matters should be addressed during scheduled sessions rather than via email.

Telehealth Disclosure
The telehealth platform used by this practice is HIPAA compliant. Confidentiality will be maintained to the extent reasonably possible; however, telehealth services carry inherent risks, including but not limited to technology failure and potential confidentiality limitations.
Clients must be physically located in the State of Colorado at the time of each telehealth session.

Privacy Policy
Please review the Privacy Policy for information regarding how personal information is collected, used, and protected.

Payment and Fees Policy
Payment for services is due at the time of each session. A valid credit or debit card must be securely maintained on file. The card on file will be charged for session fees, late cancellations, and no-show fees in accordance with this policy.
If a payment is declined or returned for any reason, you will be notified and are responsible for promptly providing updated payment information. Any outstanding balance must be resolved prior to scheduling additional appointments.
If payments are returned or remain unresolved for multiple sessions, or if a pattern of declined payments occurs, the practice reserves the right to suspend scheduling, require prepayment for future sessions, or terminate the therapeutic relationship. Outstanding balances may be referred for collection as permitted by law.
This practice operates on a private pay basis and does not bill insurance companies directly. If you wish to seek out-of-network reimbursement, a superbill may be provided upon request. Submission of superbills to your insurance carrier is solely your responsibility. Reimbursement is not guaranteed, and payment of session fees remains your responsibility regardless of insurance outcome.

Cancellation & No-Show Policy
Your appointment time is reserved specifically for you. Cancellations and missed sessions impact both the practice and other clients awaiting availability.
If the practice does not receive communication from you after three documented attempts to reach you, the therapeutic relationship may be considered terminated.

24-Hour Notice Requirement
Cancellations or rescheduling requests must be made at least 24 hours prior to the scheduled session time.

Late Cancellations
Appointments canceled with less than 24 hours’ notice will be charged the full session fee.

No-Shows
Failure to attend a scheduled session without prior notice will result in the full session fee being charged.

Insurance
Insurance companies do not reimburse for missed or late-canceled appointments. The full session fee will therefore be your financial responsibility.
For private pay clients, you have the right to receive a Good Faith Estimate explaining the anticipated cost of your care.

Emergencies
Exceptions may be made in cases of true emergencies or sudden illness at the clinician’s discretion.
By scheduling an appointment, you acknowledge and agree to this policy. A more detailed Informed Consent and Client Services Agreement will be provided prior to the initiation of services. In the event of any inconsistency between these Terms and Conditions and the signed Client Services Agreement, the signed agreement shall control.
